The Temple of Madness was featured in an old commercial of Prime Empire.
The temple appeared in one of the Arcade Pods when a police officer was taking one away.
Nya and Jay are able to get through the first part of the Temple of Madness which is a conveyer belt with rotating shapes.
After completing the restaurant, Jay and Nya come across the fire wall and inserts the three Key-Tanas. The two hug after going through so much, but in the background, an injured Sushimi is seen. He throws his malice at Jay, but Nya pushes him to the side causing Nya to take the hit. Jay laments over the loss, but is forced to move on.
Jay heads inside the throne room and meets Unagami. Jay claims Unagami is Milton Dyer, but Unagami corrects him by revealing that Milton Dyer is his creator. Unagami then uses Nya's Energy Cube to complete the Manifestation Gate. As Unagami is about to enter Ninjago, Jay pulls Unagami down believing there was a reason Dyer shut down the game. However, Unagami transform into the Empire Dragon and blazes through the portal.

Join forces with Digi Jay, Lloyd and Cole as they take on Unagami at his dangerous temple in the end-game battle of Prime Empire. Watch out as you climb the treacherous tower and try to avoid the booby-traps and get past Sushimi. At the top use all three Key-Tanas to unlock the final portal for Unagami's secret chamber. The ninjas must defeat him to complete their quest!

- This is the ninth temple to appear in the series. The first eight were the Monastery of Spinjitzu, the Fire Temple, the Temple of Light, the Temple of Fortitude, the Temple of Airjitzu, the Hanging Temple, the Oni Temple, and the Temple of Resurrection.
- The top of the temple seems to resemble a leaderboard, as it shows Unagami in first, Jay in third, and two unknown people in second and fourth respectively, named Will and Manovi. They may be the set’s designers.
